THE EVIDENCE
Coaching alongside medical care
moves the numbers you track.
TEN YEARS INSIDE THE MODEL — AND WHAT I LEARNED ABOUT COMPLEX PATIENTS
A model I know firsthand
For eight years I ran clinics for Iora Health, a primary care practice built around a collaborative team — physician, nurse, behavioral health and health coach. I saw coaching work for patients with straightforward needs and our non-clinical coaches did remarkable work.
But, I also saw the limitations for complex patients with multiple chronic conditions, shifting medications, real clinical nuance and there was a knowledge gap. A coach without clinical training can't always catch what matters or ask the right next question.
That gap is exactly what I fill: the coaching model that works, plus 30 years of nursing. Send me the patients you can't keep carrying alone.
When a Nurse leads the Coaching
Practices built on this model report real clinical movement: blood pressure control climbing from 64% to 86% of patients, with 35% fewer hospitalizations and 23% fewer ER visits. Modern Healthcare
The peer-reviewed research backs it, across coached patients in one trial, systolic blood pressure dropped nearly 22 points in six months without adding physician time.¹ And a 2023 randomized trial of nurse-led coaching, delivered one-on-one by video, produced lasting improvements in health habits for older adults with chronic conditions — gains that held even after coaching ended.² PubMed CentralSpringer
For you, that means a referral here isn't a vague wellness add-on. It's a practical extension of your care.
